2) Table data formatted with LaTeX, processed with dvi2tty.

3) Typographical error found in table 'Jun 114' in column one should read 
   'Jun 14'.  Correction made.

4) Generated TABLE.DOC, standard table description, and ABSTRACT.DOC, title and 
   abstract from the paper.
